After a hospital stay many people find that they need additional time to recuperate as they transition back to home or other community based settings. The Post Acute environment is designed exclusively to address this need by providing 24 hour skilling nursing and rehabilitation services. Our services include:

In our Post Acute environment, we provide large spacious private rooms, an expansive rehabilitation gym and an individualized focus provided by a highly qualified professional staff. These are elements that set us up apart from a traditional skilled nursing facility. We will allow our patients to rehabilitate in a private setting around others who share the same goals of a short term rehab stay. Kissito holds quality of life in the highest regards as we strive to provide care and betterment of human life.

Rehabilitation

• We employ physical, speech and occupational therapists who are focused and trained on meeting the specific needs of the older adult who desires to return home. The dedicated rehabilitation manager at each of our homes personally coordinates rehabilitative care plans, assures compliance with all insurance requirements, and ensures that each resident and/or family member is an active participant in rehabilitation decisions.
• All of our facilities contain a rehabilitation gym well-equipped to handle the varying needs of residents. Our rehab gyms are active and energetic areas providing an uplifting and healing atmosphere for our residents. We serve patients with a multitude of diagnoses. The most common rehabilitation diagnoses include stroke, orthopedic conditions like hip and knee replacement, arthritis, and spinal cord injuries. Most patients are admitted directly from a hospital's medical/surgical unit, but patients can be admitted from any level of care, as well as home. Our team works closely with our residents' physicians, designing and implementing individualized treatment programs.

Medically Complex Care

• Our facilities play an important role in the healthcare of the communities we serve, taking patients when they can no longer remain in the fast-paced hospital environment and need an affordable and comfortable place where they can rehabilitate before returning home. We offer advanced clinical care for the medically complex patient; our nursing staff is highly trained to handle a variety of complex procedures. We understand that many of our patients want to go home and that they just need a little extra time and assistance getting there... at Kissito we help every step of the way, from admission to the trip home.
• Our expert skilled nursing staff works 24 hours a day to monitoring complex medication condition and ensuring that medication, diagnostic, nutrition and rehabilitation needs are met. Our medially complex care services include wound care, pain management, diabetic management and a variety of discharge and diagnosis management education.

Sub Acute Care

• Our homes play an important role in the healthcare of the communities we serve, taking patients when they can no longer remain in the fast-paced hospital environment and need an affordable and comfortable place where they can recuperate before returning home.
• Our expert skilled nursing staff works round the clock to monitor and care for our residents' conditions, ensuring medication, diagnostic, nutrition and rehabilitation needs are met.
• Our medical care rivals the care received in hospitals, and at Kissito our residents are not just a medical record number... we know our residents and their friends and family by name, we know what our residents like to eat, how they enjoy passing their time, and other important details that enhance quality of living.
• We know our sub acute residents want to go home and that they just need a little assistance getting there... at Kissito we help every step of the way, from admittance to the trip home.

Palliative Care

• We are focused on compassionate and loving end of life care, and providing physical, emotional and spiritual support to the resident and their family and friends.
• We respect our residents' rights to understand their medical condition and to participate in and choose their medical care, and work closely with our residents and/or designated family members and physicians to design an environment and plan of care that will ease pain and give peace of mind.

Special Care

• We provide special care for residents afflicted with Alzheimer's or other conditions affecting memory loss or orientation.
• We understand that dementia affects individuals in different ways, and our specially trained nursing, therapy and activities staff is committed to the on-going assessment of each residents' needs, and designing care plans specifically designed to meet those needs and optimize our residents' life experience.
• Many of our homes offer secured units designed to enhance quality of life in a safe and pleasant environment. These units offer peaceful decor and safe rooms and hallways where residents can continue to enjoy an active and meaningful lifestyle.
• Currently our homes in Hot Springs, VA and Yuma, AZ have secured dementia units. Our home in Hondo, TX is in the process of making the addition as well, and we hope to offer secured units in our other homes in the future. In our homes without a secured unit, we still provide the same specialized care and activities for our Alzheimer's and dementia residents.

Medicare / Medicaid Provider

• We are a Medicare and Medicaid provider, and also partner with many commercial and managed care insurance providers.
• We understand that dealing with insurance information can be confusing. Our experienced staff of financial professionals is always happy to assist with understanding and navigating the complex world of medical reimbursement.
• There is no question too large or small for our team. We welcome the opportunity to serve our residents and their families to ensure they are comfortable with their medical care and the costs associated with their stay at our homes.
