Kissito utilizes the non-acute Skilled Nursing Facility (“SNF”) provider platform as a vehicle to engage patients and caregivers in Patient Self Management Skills. Elders receive up to 100 days of SNF benefits after a three-day prequalifying acute hospital stay. The SNF stay, focusing on recuperation and rehabilitation, should average about 37 days addressing diseases or events such as CHF, Heart Attack, Stroke, Pneumonia, Hypertension, Falls with Injury, Orthopedic Surgery, and Diabetes. Most SNF patients have multiple complexities and a high frailty level. The 37 day SNF stay presents one of the health systems only true opportunities to engage the patient and their care giving team in a structured health awareness and education program.
