This treatment based model has led to a setting based payment system which leaves little incentive for the efficient delivery of services. For several years, Kissito has been seeking systemic improvements in patient outcomes while reducing cost through our innovative approach to caring for America’s Elders. We seek to revolutionize the care of our Elders through the integration of patient self management skills, non-acute management of chronic disease, Kissito at Home, and technology accelerators.

Our experience in global health has reinforced the critical importance of health education and disease self awareness in the efficient management of a country’s disease burden. Sadly, in America we do not fully reimburse providers for such education and awareness and, therefore, the level of Chronic Disease understanding and patient self management skills amongst America’s elderly is almost non-existent. In collaboration with researchers from Albert Einstein College of Medicine (“AECM”), Kissito has implemented comprehensive patient self management skills assessment tools and educational programs to enhance such skills. The evidence is clear — after Kissito’s intensive interventions, both patients and care givers are discharged to home with a much broader understanding of effective self management. Patients and care givers are trained in such areas as disease awareness, red flag events, nutrition, medication self-administration, health practitioner communications, and motivational goal setting. Discharge assessments show significant improvements in patient and care givers self management skills. Kissito is currently piloting an internet-based module of its self management program which provides the patient and care giver with a personal medication administration record and a comprehensive set of self management tools.
